Circle of Hope Legacy Society
Naming HopeHealth as a beneficiary in your estate plan helps to ensure that HopeHealth will be ready to provide compassionate care to patients and families facing serious illness and loss far into the future.
The Circle of Hope Legacy Society recognizes generous donors who have included a gift to HopeHealth in their will or trust or have named HopeHealth as a remainder beneficiary. Indeed, when you make such a generous gesture, you are in good company, standing with countless other Legacy Society members - caring individuals who have recognized HopeHealth’s compassionate skill and caring to be a vital resource for those facing serious illness and loss when time matters most.
Membership in the Legacy Society is entirely voluntary, and there is no immediate gift, minimum gift amount, or minimum age required to become a member.
As a valued member, HopeHealth is proud to:

To become a member of the Circle of Hope Legacy Society, simply notify HopeHealth of your intentions. It’s as easy as completing and returning this simple confidential form. When you do, we will welcome you as a member of the Legacy Society and be able to personally thank you. Providing a few simple details about your gift and sharing your vision of how you would like your gift to support HopeHealth’s future will ensure that the intent of your gift will be honored in the future.
